29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a) Throughout the job site - On or about March 18, 2021 the employer did not ensure the employee operating the powered industrial vehicle (PIV) was trained and certified, exposing employees to struck by hazards.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Top of Building C and E - On or about March 18, 2021 the employer did not ensure each employee engaged in roofing activities above 6 feet were utilizing a means of fall protection, exposing employees to fall hazards.
