29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. At the jobsite, employees operating the Dewalt 18" cut off saw while cutting 5ft. long rebar were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, the lower blade guard did not automatically and instantly return to the covering position, when such guards are in use. At the jobsite, employees operating a Dewalt 18" cut off saw with the lower level guard wedged in the up position while cutting 5ft. long rebar material. The employees were exposed to amputation hazards.
