29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   Ohio Asphalt Roofing, Inc. at the worksite located at 164 Chelsea Ct. in Toledo, Ohio: On or about March 24, 2021, the employer did not ensure employees were protected from falls of approximately nine feet while performing roofing operations on a residential structure.   Ohio Asphalt Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1263098, citation number 1, item number 1a and was affirmed as a final order on October 11, 2017, with respect to a workplace located at 7261 Saint Thomas Ct., Toledo, OH 43617.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
