29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: At a residential worksite located at 8533 N McKinley Ave, Kansas City, Missouri. Employees were exposed to a fall of 18 feet to he surface below when using a JLG front end loader to support a scaffold that was not signed by the manufacturer for the purposed it was being used.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At a residential worksite located at 8533 N McKinley Ave, Kansas City, Missouri- Employees were performing roofing operations at an elevated height on a steep pitch roof without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR  1926.501(b)(14):Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. At a residential worksite located at 8533 N McKinley Ave, Kansas City, Missouri. Employees were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 13 to 14 ft. fall hazard to the concrete surfaces below.

29 CFR  1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, shall be equipped with: At least one handrail; and One stairrail system along each unprotected side or edge.Stairways were not equipped with at least one handrail and one stairrail system as required.  At a residential worksite located at 8533 N McKinley Ave, Kansas City, Missouri.  Employyes were exposed to serious slip, trip and fall hazards while using stairways from the basement to the first floor and from the first floor to the second.  The stairways lacked handrails and no stair rail systems were in place on unprotected sides.
