29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards. A safety and health program with policy, hazard assessment, and site work rules was not developed and/or implemented at this job site.    a) Jobsite located at 3829 Lancaster, Philadelphia, PA: The employer had not initiated, developed or implemented a safety and health program to include, but not limited to, identification and control of hazards, training and education, and work safety rules and procedures to enable employees to recognize and avoid unsafe conditions and hazards.  Condition was observed on March 29, 2021.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) Jobsite located at 3829 Lancaster, Philadelphia, PA: Employees were working from the upper level platform of a tubular welded frame scaffold, that did not have guardrails installed on all open sides, while not utilizing a fall protection system thereby exposing employees to fall hazards of greater than 28 feet.   Condition was observed on March 29, 2021.

Condition was observed on or about March 29, 2021.
