Overland Park, KS, employees engaged in residential roofing activities were exposed to fall hazards of approximately 19 feet from the roof to the ground in that fall protection was not utilized.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At jobsite located at 14115 Mohawk Rd. Overland Park, KS, employee engaged in residential roofing activities were exposed to falls from approximately 12 feet, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof nor was secured to structure.
