29 CFR 1926.501(b)(4):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) Jobsite located at 2235 Castor Avenue, Philadelphia, Pa - The employer did not ensure employees were protected from falling through holes on the roof. Employees removing old roofing material and disposing the materials through the holes were exposed to falls of approximately 21 to 25 feet.   Condition was observed on or about 29 MAR 2021.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) Jobsite located at 2235 Castor Avenue, Philadelphia, Pa -The employer did not ensure that employees utilized fall protection. Employees were engaged in roofing activities approximately 25 feet above the ground level while working on a commercial building, thus exposing employees to fall hazards.  Condition observed on or about 29 MAR 2021.  Green Bay Construction, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1255607, citation number 1, item number 1 and was affirmed as a final order on12/26/2018, with respect to a workplace located at 67-71 Somme St.  Newark, NJ .  Green Bay Construction, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1413126, citation number 1, item number 2 and was affirmed as a final order on 01/29/2020, with respect to a workplace located at 6958 Torresdale Avenue, Philadelphia, Pa.

Employees were working on roof approximately 25 feet above the ground level. Employees were not trained on how to recognize, evaluate, and mitigate hazards associated with working on a roof.   Condition was observed on or about 29 MAR 2021.  Green Bay Construction, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1413126, citation number 1, item number 3 and was affirmed as a final order on 01/29/2020, with respect to a workplace located at 6958 Torresdale Avenue, Philadelphia, Pa.
