29 CFR  1926.451(b)(2):Scaffold platforms and walkways were not at least 18 inches wide:  PFMW Edwardsville LLC dba Planet Fitness located at 6483 Center Grove Rd, Edwardsville, IL, 62025:   On or about 4/1/21, the employer did not ensure that maintenance employees conducting window cleaning operations inside the facility, above ten feet in a scaffold (to include scissor lift) were on platforms and walkways at least 18 inches wide.  Employees used guardrails and bracing to climb in and out of the scissor lift basket, and to reach windows to be cleaned. Rails and bracing were less than 18 inches wide.
