29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge, which was 6 feet (1.8 m) or more above a lower level, was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about April 5, 2021, at the above addressed jobsite, employees performing residential roof demolition operations, eleven (11) feet five (5) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards. b. On or about April 5, 2021, at the above addressed jobsite, employees performing residential roof demolition operations, eleven (10) feet five (7) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about April 5, 2021, at the above addressed jobsite, the portable ladder used to access and egress the roof was extended two (2) feet above the upper landing surface, thereby exposing an employee to fall hazards.
