On or about April 6, 2021, on the southwest side of the site; an employee was lifted while standing inside an excavation bucket that was being lowered into the excavation trench.

29 CFR  1926.501(b)(1):):"Unprotected sides and edges."  Employees on walking/working surfaces with an unprotected side and/or edge which is 6 feet or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   On or about April 6, 2021, on southwest side of site, an employee  was observed standing on top and subsequently riding a fuel storage tank approximately seven (7) feet in height while being hoisted by the excavator from ground to bottom of the excavation without adequate fall protection.

29 CFR  1926.501(b)(7)(ii):Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers.  On or about April 6, 2021, southwest side of site employees worked around the edge of an open excavation that was over six (6) feet in depth with no barriers, guardrails, or personal fall protection. The excavation was approximately seventeen feet in depth and seventeen feet in width.

On or about April 6, 2021, Southwest side of the job site, employees entered an excavation approximately seventeen (17) feet in deep in order to install an underground fuel storage tank, without a means of safe egress.

On or about April 6, 2021, on the southwest side of the site employees installed an underground fuel storage tank while in an excavation at a depth of approximately seventeen (17) feet deep and seventeen (17) feet wide without an adequate protective system..
