At the facility located at 140 Stockton Street in Jacksonville, Florida: On or about April 6, 2021, the employer exposed employees to struck-by and crushing hazards, in that several of the steel storage rack system columns were inadequately anchored to the floor, in that not all column base plates were properly bolted to the slab or subgrade, as employees utilized the rack systems for the storage of materials.  c. At the facility located at 140 Stockton Street in Jacksonville, Florida: On or about April 6, 2021, the employer exposed employees to struck-by and crushing hazards, in that several of the steel storage system racks were damaged, thereby reducing the structural capacity of the racks that employees utilized for the storage of materials.
