On or about April 5, 2021, at 11055 Pine Acre Trails Blvd. in Conroe, TX, during the construction of an elevated water storage tank, the employer did not ensure that the anchorage used for the employees personal fall arrest system, was capable of supporting at least 5,000 pounds per employee and maintain a safety factor of at least two as an anchorage point.
