29 CFR 1917.93(a): The employer did not ensure that each affected employee wore a protective helmet when working in areas where there is a potential for injury to the head from falling objects. a) F/V Dunedin Star - On or about March 30, 2021, an employee performed an inspection of the crews identity accessed the vessel during longshoring operations. The employee walked aboard the vessel without the use of head protection and was exposed to overhead hazards from nearby crane operations.

The employee walked aboard the vessel without the use of a personal flotation device and was exposed to drowning hazards.
