29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Gustabo Sanchez located at 298 Baldwin Ct, Westerville, OH 43082: On and preceding April 7, 2021, employees were performing roof work on a two story residential home at heights up to 22 feet without fall protection, PPE and improperly configured ladders. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

The following condition exposed employees to struck-by hazards to the eyes.

Roofers were installing shingles and other roofing materials at heights up to 22 feet without fall protection. The following condition exposed them to falls up to 22 feet.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Gustabo Sanchez located at 298 Baldwin Ct, Westerville, OH 43082: On and preceding April 7, 2021,  the employer did not ensure that the two ladders used for access to the roof extended at least three feet beyond the surface they were accessing. Roofers were using two extension ladders to access the roof of a two story residential home with the both ladders extended 10 inches beyond the eave and was not secured to the house. The following condition exposed them to falls up to 9 feet.
