29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees engaged in residential construction activities at heights greater than six (6) feet above the ground were not provided training in the recognition of fall hazards and methods to reduce the hazard of falling from heights.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Back of house:  On 08 April 2021, an employee installing roof sheeting slipped and fell approximately ten (10) feet from the roof to the ground where fall protection was not used. (b) Side of house:  On 09 April 2021, an employee was working on the roof near the edge at the peak more than eighteen (18) above the ground below without fall protection exposing him to falls.
