29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) On or about April 12, 2021, Enviro Tech International Inc., did not ensure a written respiratory protection program with worksite specific procedures was established for employees who were required to use 3M 7000 series full-mask dual cartridge type respirators when performing mixing and filling  operations for 1-bromopropane (n-propyl bromide).  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used:  a) On April 12, 2021, Enviro Tech International Inc., did not ensure that employees were fit tested prior to requiring employees to use 3M 7000 full face respirators when performing mixing and filling operations for 1-bromopropane (n-propyl bromide) in the mixing room. The employee was fit-tested with a half-face respirator.  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Employee(s) who performed 1-bromopropane (n-propyl bromide), mixing and filling operations, were required to use 3M 7000 series full-mask dual cartridge type respirators.  In accordance with 29 CFR 1903.19(c), abatement certification only is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
