Employees were exposed to formaldehyde at concentrations greater than the OSHA Short-Term Exposure Limit (STEL) of two parts per million (2.0 ppm) of air averaged over an fifteen-minute period: At the jobsite located at 8201 East 34th Circle North, Wichita, KS, employee engaged in grossing activities was exposed to an 15-minute time weighted average (15-min TWA) level of 2.50 ppm of formaldehyde, which is 1.25 times the OSHA Short-Term Exposure Limit (STEL) of 2.0 ppm.

29 CFR 1910.1048(e)(1)(i): The employer shall establish regulated areas where the concentration of airborne formaldehyde exceeds either the TWA or the STEL and post all entrances and access ways with signs bearing the following legend: DANGER FORMALDEHYDE MAY CAUSE CANCER CAUSES SKIN, EYE, AND RESPIRATORY IRRITATION AUTHORIZED PERSONNEL ONLY At the jobsite located at 8201 East 34th Circle North, Wichita, KS, employees engaged in grossing activities were exposed to formaldehyde which is a carcinogen, skin sensitizer, causes skin irritation, eye damage and it's also a specific target organ toxic in a single exposure. Employer did not establish regulated areas where the concentration of airborne formaldehyde exceeded either the TWA or the STEL and/or did not post all entrances and access ways with signs as required.

Employer failed to ensure the use of goggles and/or face shields while employees perform grossing activities in the grosser room. Employees were exposed to a chemical mixture containing 10% formaldehyde.
