29 CFR 1926.1053(b)(13): The top or step of a stepladder was used as a step:  On or about 4/06/2021, at the jobsite located at 4161 NW 60th Circle Boca Raton, FL 33486,  an employee was performing repairs to a metal screen roller from an 8-foot fiberglass Little Giant 22 feet Velocity aluminum multi-ladder leaning against the wall while standing at the ladder's second top step, exposing him to a 15-feet fall hazard.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about 4/06/2021, at the jobsite located at 4161 NW 60th Circle Boca Raton, FL 33486, the employer did not train employees in the safe usage of ladders, exposing employees to a15-feet fall hazard while using a Little Giant 22 feet Velocity aluminum multi-ladder leaning against the wall, while performing repairs to a metal screen roller.
