29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Jobsite:  The owner did not maintain a written certification of fall protection training for the employee who was exposed to fall hazards while installing the new roof.  On or about 10 March 2021, the employee fell from the roof while in the prosses of installing new metal roofing panels and sustained serious injuries..

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  Jobsite: The employer did not report the in-patient hospitalization of an employee within twenty-four (24) hours to OSHA.  On or about 10 March 2021, the employee fell from the roof while in the prosses of installing new metal roofing panels and sustained serious injuries.
