29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  (a) Center for Social Dynamics, LLC at 2500 Arapahoe Ave in Boulder, CO: On or before April 8, 2021, the employer did not verify that the required workplace hazard assessment had been performed through a written certification. Employees performed behavioral therapy in client homes, working with children and adults with developmental delays, and facing physical aggression from clients, such as biting and scratching. The employer did not determine if hazards were present, or were likely to be present, which necessitated the use of PPE, such as exposure to workplace violence and bloodborne pathogens. This exposed employees to safety and health hazards.

29 CFR  1910.1030(c)(1)(ii)(B): The employer's Exposure Control Plan did not include the schedule and method of implementation for 29 CFR 1910.1030(f), Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up:  (a) Center for Social Dynamics, LLC at 2500 Arapahoe Ave in Boulder, CO: On or before April 8, 2021,  the employer did not ensure the Exposure Control Plan (ECP) contained the schedule and method of implementation for providing the Hepatitis B vaccination to exposed employees. The employer's ECP determined that all employees had occupational exposure to bloodborne pathogens. Employees provided in-home behavioral therapy services to clients who were aggressive and occasionally bit employees. This exposed employees to a bloodborne pathogens hazard.

The employer determined that all employees had occupational exposure to bloodborne pathogens.
