On April 15, 2021 at W6464 Sunny Drive in Menasha, Wisconsin, the employer did not ensure safety training programs had been initiated prior to employees installing roof materials on a two-story building including fall safety and ladder safety programs.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On April 15, 2021 at W6464 Sunny Drive in Menasha, Wisconsin, the employer did not provide required fall protection safety training for employees exposed to fall from elevation hazards when working on a roof surface with unprotected edges at greater than six feet above surface level without utilizing conventional fall protection equipment.
