29 CFR 1904.29(b)(3): The employer failed to enter each recordable injury or illness on the OSHA 300 Log within seven (7) calendar days of receiving information that a recordable injury or illness had occurred. On or about April 20, 2021, the Thayer Ford facility at 18039 N. Dixie Highway, Bowling Green, Ohio did not record a recordable injury in the OSHA 300 Log within seven (7) days of first learning about the incident, instead entering the recordable injury into the 300 Log seventeen (17) days after the employers received information about the incident.

29 CFR 1904.39(a)(2): The employer failed to report an in-patient hospitalization to OSHA within twenty-four (24) hours after the in-patient hospitalization of an employee as a result of a work-related incident. On or about April 20, 2021, The Thayer Ford facility located at 18039 N. Dixie Highway, Bowling Green, Ohio did not report an employee in-patient hospitalization within twenty-four (24) hours after the employee was hospitalized as a result of a work-related incident. Instead the employer reported the hospitalization approximately four (4) days after first learning about the in-patient hospitalization.
