(a) Hoying Construction, Inc., on or about April 20th, 2021, at the location of 158 Dickman Rd., Saint Marys, OH, 45885: Employees were performing roofing work on top of a new residential structure and accessing the roof using a portable ladder that was not extended 3 feet above the roof.  Employees were exposed to fall hazard of approximately 12 feet to the ground below without the benefit of accident prevention programs to identify and mitigate these hazards.

Employees were exposed to fall hazards of approximately 12 feet to the ground below while transitioning between the ladder and the roof.
