29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection system, such as a personal fall arrest, travel restraint, or positioning system:  a.  On or about April 20, 2021, at the tipping floor of the transfer station, the employer did not ensure that employees required to place a cover tarp on trailers filled with refuse were protected from a 13 feet fall hazard, in that employees regularly stood on the concrete pit guardrail to grab and move the tarp.
