29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):   6300 St. Louis Avenue, St. Louis, MO 63121:  Employer is exposing employees to hazardous conditions where the employees are permitted to operate forklifts which has some  defects. On 4/15/2021 an employee was observed operating a forklift (Clark Model C25L, S/N P232L08469839CNF) which had a broken seat that hung on a piece of wood and a brake pedal which did not have a slip resistance pad.   The forklift was not removed from service until the defects were corrected.

29 CFR  1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219:   6300 St. Louis Avenue, St. Louis, MO 63121:  Employer is exposing employees to hazardous conditions where the employees are permitted to operate a piece of equipment  which does not have the transmission apparatus enclosed.   About 4/15/2021 employees operated a piece of equipment used for grinding wood pallets. The equipment did not have the belts and the pulleys enclosed.  The employees were exposed to the hazards of moving parts.

The employees are permitted to operate a pallet tear down piece of equipment which is not guarded to prevent employee contact with rotating edges of a blade. On 04/15/2021 employees were exposed to unused run of the blade of a horizontal tear down saw (Long saw) which was not guarded.

The employees are permitted to operate electrical equipment which has exposed energized internal parts. On 4/15/2021 an employee was exposed to electrical hazards where the employee operated a fan and a radio powered from a duplex electrical equipment which had exposed energized internal parts.
