29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): 6300 St. Louis Avenue, St. Louis, MO 63121: Employer is exposing employees to hazardous conditions where the employees are permitted to operate forklifts which has some defects. On 4/15/2021 an employee was observed operating a forklift (Clark Model C25L, S/N P232L08469839CNF) which had a broken seat that hung on a piece of wood and a brake pedal which did not have a slip resistance pad. The forklift was not removed from service until the defects were corrected.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219: 6300 St. Louis Avenue, St. Louis, MO 63121: Employer is exposing employees to hazardous conditions where the employees are permitted to operate a piece of equipment which does not have the transmission apparatus enclosed. About 4/15/2021 employees operated a piece of equipment used for grinding wood pallets. The equipment did not have the belts and the pulleys enclosed. The employees were exposed to the hazards of moving parts.
