29 CFR  1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system.  a. Blender Platform  - On or about April 20, 2021 a staircase was removed from the blending platform and a chain was used as a guardrail for the opening.  The chain was slack and sagged downward in the center and the middle chain was connected to the top chain with a bolt and nut which eliminated the mid rail.  The employees were exposed to a fall hazard due to the improper installation of the chain.
