29 CFR  1910.146(k)(3)(ii): Whenever an authorized entrant entered a permit space, a mechanical device was not available to retrieve personnel from vertical type permit space more than 5 feet (1.524 m):  On or about April 20, 2021, and at times prior thereto, In the EO blast room, employees worked in permit required confined spaces that had vertical entrances with heights greater than 5 feet without having a mechanical device available to retrieve personnel.

29 CFR  1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    On or about April 20, 2021, and at times prior thereto, In the EO blast room, employees were working near an exposed shaft next to the stairway of the EO blast room platform without a guard to protect employees from the hazards associated with making contact to a rotating shaft.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed:  On or about April 20, 2021, and at times prior thereto, In the EO blast room, wires used to power an actuator switch for abrasive blasting equipment were not protected from abrasion and the opening where the conductor wires entered the metal box was not effectively closed.
