29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  A).	On or about 04/23/2021, at the site of 43-26 219th Street. Bayside, NY  Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and materials from other employees who on the pitch roof throwing them down. Protective helmets were not worn.

On or about 04/23/2021, at the site of 43-26 219th Street. Bayside, NY  Employees installing siding/roof on a pitch roof with a slope of 3 in 12 ratios were exposed to the hazard of falling approximately 11 ft. to the ground below.
