29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A)  Winzeler Stamping Company, 910 E. Main Street, Montpelier, Ohio 43543,  On April 20, 2021, the employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work the 1910.147(d)(2) requirements.  Employees were exposed to machine hazards which resulted in an employee suffering an  amputation of the left pinky finger.

29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:  A) Winzeler Stamping Company, 910 E. Main Street, Montpelier, Ohio 43543,  On April 28, 2021, the employer failed to ensure that the OSHA Form 300 for calendar year 2021 contained a complete description in column F.  The 2021 300 log did not define the specific injury sustained by the employee.
