29 CFR  1915.89(c)(1):  The employer did not ensure the use of a lock to prevent energization or startup, or the release of hazardous energy, before any servicing was started:  a) Jobsite located at 5195 South 19th Street, USNS Peary Engine Room, Switchboard Room 3-72-0, "Circuit Breaker-5", Philadelphia Naval Yard, Philadelphia, PA.: Employee(s) were exposed to electrical hazards from an energized circuit breaker (6600kV), on a switchboard that was not tagged out, de-energized, restrained, or isolated from all energy sources of power. No other verification of additional safety measures necessary to provide the equivalent safety available from the use of a lock were implemented.  Condition was observed on or about 28 APR 2021.
