29 CFR  1926.451(e)(8):Direct access to or from another surface shall be used only when the scaffold is not more than 14 inches (36 cm) horizontally and not more than 24 inches (61 cm) vertically from the other surface.  The employer is failing to protect an employee from fall hazards while accessing and egressing a roof from scaffolding. This was most recently observed at 2323 S 144th St, Omaha, NE 68144 on April 14, 2021 where the employer has an employee directly accessing a roof from a scaffold that is more than 14 inches horizontally from the other surface.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.  The employer is failing to protect an employee from fall hazards while performing roofing activities. This was most recently observed at2323 S 144th St, Omaha, NE 68144 on April 14, 2021 where the employer has an employee working directly adjacent to an unprotected edge of a walking-working surface.
