29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  At the construction site located at 4151 Olive St. in Kansas City, Missouri:  Employees engaged in framing activities were exposed to fall hazards of approximately 13 feet from the edge of the level they were working on to the exterior ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the residential construction site located at 4151 Olive St in Kansas City, Missouri: Employees were exposed to fall hazards. Employees were observed accessing and egressing the roof by way of portable ladders that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 13 feet eave to ground.
