29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:  At a residential worksite located at 721 N Rockwell Ave, Independence MO -- The employer did not train employees in hazard recognition, including but not limited to conditions where surfaces may not support body weight, or where fall protection is required. The employer does not have a safety training program. This was documented on or about 04/30/2021.

29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a residential worksite located at 721 N Rockwell Ave, Independence MO -- employees were on a deck that had unprotected sides/rails and were not using any kind of personal fall arrest system. This exposed employees to fall hazards greater than 6 feet above ground level. This was documented on or about 04/30/2021.
