29 CFR  1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  (a) On or about 4/18/2021 - Moundville, AL, onboard the material/hopper/rock barge,  employees were exposed to struck-by and crushing hazards from the gravitational energy associated with the elevated bucket of a Case Front End Loader (Model 621F, Serial Number NEF221458) while propping the bucket against the side of the barge to replace a leaking O-ring.  (b)  On or about 04/22/2021 - Moundville, AL, onboard the material/hopper/rock barge, employees were exposed to struck-by and crushing hazards from the gravitational energy associated with the elevated bucket of a Case Front End Loader (Model 621F, Serial #NEF221458) while propping the bucket against the side of the barge to replace a leaking O-ring.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  (a) On or about 4/18/2021 - Moundville, AL, onboard the material/hopper/rock barge, employees were exposed to struck-by and crushing hazards from the gravitational energy associated with the elevated bucket of a Case Front End Loader (Model 621F, Serial Number NEF221458) in that affected employees performed the duties of authorized employees when they performed tasks that required lock out  to   isolate the gravitational energy to replace an O-ring.  (b) On or about 04/22/2021 - Moundville, AL, onboard the material/hopper/rock barge,  employees were exposed to struck-by and crushing hazards from the gravitational energy associated with the elevated bucket of a Case Front End loader (Model 621F, Serial Number NEF221458) in that affected employees performed the duties of authorized employees when they performed tasks that required lock out to isolate the gravitational energy to replace an O-ring.
