On May 5, 2021, the employer did not establish and implement elements of a written respiratory protection program for employees voluntarily wearing Honeywell Series 7700 elastomeric half-face respirator, with HEPA/organic vapor cartridges, while performing the lead casting operation, to ensure they were medically able to use it.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

On June 10, 2021, the employer did not ensure that employees stored their respirators to protect them from contamination of chemical. A half face tight-fitting negative pressure respirator was stored  inside a tool cart without a proper storage container. The tool cart was located in the shop floor were the casting and machining of lead is preformed. A wipe sample collected from the inside of an employee respirator indicated the presence of lead.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
