29 CFR  1910.134(c)(2)(ii):The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a) Donlon Manufacturing, LLC located at 125 Elizabeth Drive, Arlington Heights, IL 60005.  On May 5, 2021, the employer did not establish and implement elements of a written respiratory protection program for employees voluntarily wearing Honeywell Series 7700 elastomeric half-face respirator, with HEPA/organic vapor cartridges, while performing the lead casting operation, to ensure they were medically able to use it.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.134(h)(1)(i):Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:  a) Donlon Manufacturing, LLC located at 125 Elizabeth Drive, Arlington Heights, IL 60005.  On June 10, 2021, the employer did not ensure that an employee cleaned and disinfected half face tight-fitting negative pressure respirator as often as necessary to be maintained in a sanitary condition. A wipe sample collected from the inside of an employee respirator indicated the presence of lead.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.134(h)(2)(i):Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a) Donlon Manufacturing, LLC located at 125 Elizabeth Drive, Arlington Heights, IL 60005.  On June 10, 2021, the employer did not ensure that employees stored their respirators to protect them from contamination of chemical. A half face tight-fitting negative pressure respirator was stored  inside a tool cart without a proper storage container. The tool cart was located in the shop floor were the casting and machining of lead is preformed. A wipe sample collected from the inside of an employee respirator indicated the presence of lead.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:  a) Donlon Manufacturing, LLC located at 125 Elizabeth Drive, Arlington Heights, IL 60005.  On May 5, 2021, the employer did not maintain all surfaces as free as practicable of accumulations of lead. Wipe sample collected on May 5, 2021 in the lunchroom from the microwave keypad, the lunchroom door-knob, the water fountain handle; and in the bathroom from the locker door handle indicated the presence of lead.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Donlon Manufacturing, LLC located at 125 Elizabeth Drive, Arlington Heights, IL 60005.  On May 5, 2021, the employer did provide employees wearing respirators for voluntary use with the information contained in Appendix D of 29 CFR 1910.134.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
