29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about May 5, 2021, the employer did not ensure an authorized employee performing a die change on the 2236 Mechanical Hot Trim Press followed the established energy control procedures by; isolating the machine from the mechanical and gravity energy source(s) [per 1910.147(d)(3) requirements], affixing a lockout device to the energy isolating devices [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)].

29 CFR  1910.218(f)(1)(iii):The ram shall be blocked with a material the strength of which shall meet or exceed the specifications or dimensions shown in Table O-11.  On or about May 5, 2021, the employer did not ensure the ram of 2236 mechanical hot trim press was blocked when a die was changed.  Sifco Industries, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.218(a)(3)(iv), which was contained in OSHA inspection number 1303946, citation number 2, item number 1 and was affirmed as a final order on July 23, 2018, with respect to a workplace located at 970 East 64th Street, Cleveland, Ohio, 44103.
