29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment.  CNC Machine Area:  On or about May 06, 2021, the employer did not determine the hazards present and what personal protective equipment is needed for handling an injurious corrosive material of distillates (petroleum), hydrotreated heavy naphthenic chemical that is in the Tech Cool 35052CF coolant employees' use weekly. Employees do not wear the necessary personal protective equipment, per the safety data sheet, risking exposure to skin irritation and sensitization and serious eye damage while they transfer the coolant from a 55-gal drum to their respective machines in the CNC Machine Area.

Company-wide:  On or about May 06, 2021, the employer did not develop a written hazardous communication program to address the hazardous chemicals and material that is stored, used, milled, grinded, and cut in the facility. Some of the chemicals stored in the flammable cabinet included a category 1B carcinogen/category 1 flammable aerosol Rustoleum Spray Paint and a category 1 flammable aerosol/category 2 carcinogen Krylon Spray Paint + Primer located in the Tool Room. Employees also mill A2 Rod steel that contains less than five percent of carbon, lead silicon, and chromium metals.

29 CFR  1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were prominently displayed on the container or readily available in the work area throughout each work shift.  Tool Room:  On or about May 06, 2021, the employer did not ensure that all containers stored in the flammable cabinet prominently displayed all the labels of the products. Labels on some of the containers were covered with the product preventing employees from clearly determining what it was and what the dangers of handling it was.

29 CFR  1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  Tool Room:  On or about May 06, 2021, the employer did not remove from service a damaged extension cord that was plugged into an outlet behind the OD grinder in the Tool Room Area. Employees were exposed weekly to electrical fire hazards caused by damaged extension cords when they used the OD grinder.
