29 CFR 1910.134(k): The employer did not provide the information on respirators in accordance to Appendix D of the section to employees who wear respirators when not required by this section or by the employer to do so:   On or about May 05, 2021, and at times prior thereto, at a workplace located at 1401 S. Riverside Drive, Fort Worth, Texas; the employer did not ensure that employees using 3M N95 particulate respirators were provided copy of Appendix D. Employees were potentially exposed to respiratory hazards when performing tasks such as, but not limited to calcium chloride application.

29 CFR 1910.1200(h)(1):  Employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g. flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets:                                                                                                              On or about May 05, 2021, and at times prior thereto, at a workplace located at 1401 S. Riverside Drive, Fort Worth, Texas; the employer did not provide employees with chemical specific information and training on hazardous chemicals being used daily in the calcium chloride process area exposing employees to health hazards.
