29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  (a)	RK Steel, LLC at 8221 East 96th Ave, Henderson, CO 80640:  On and before April 29, 2021 the employer did not ensure that the Lown Slip Roll Forming and Plate Bending Machine, Model B-798 and serial number B7066, was effectively guarded.  An employee was bending a steel panel, which was approximately 14-inches by 11-inches, when one of his gloves got caught on a bur on the panel and pulled his finger in between the two ingoing rollers.  The employee sustained a fingertip amputation injury.  This condition exposed employees to laceration and amputation hazards.
