29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.    On or about May 10, 2021, the employer did not ensure a safe means to egress was provided to an employee working within a trench. This exposed the employee to a fall hazard.

29 CFR  1926.651(j)(2):  Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.   On or about May 10, 2021 the employer did not ensure an employee was protected from excavated materials while working within a trench.  This exposed the employee to struck-by hazards.

29 CFR  1926.652(b)(3)(i): The designs of sloping or benching systems were not selected from and in accordance with tabulated data, such as tables and charts.    On or about May 10, 2021, the employer did not ensure that the benching system was in accordance with the tabulated data and the tables and charts that illustrate the proper way to bench and slope an excavation.  This exposed the employees to cave-in hazards.
