29 CFR  1926.451(e)(7):Steps and rungs of ladder and stairway type access did not line up vertically with each other between rest platforms:  On May 9, 2021 and at times prior to, in the center of commercial building under construction, employees accessed a scaffold platform located 20 feet above ground utilizing integral prefabricated scaffold access frames that did not line up vertically, exposing the employees to a 20-foot fall hazard.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On May 9, 2021 and at times prior to, in the center of commercial building under construction, the top level of the supported scaffold used by employees to weld steel purlins on the bottom of the steel roof truss did not have a  guardrail system installed, exposing the employees to a 20-foot fall hazard.

On or about May 9, 2021 and at times prior to, in the center of commercial building under construction, employees working from a fabricated framed scaffold were exposed to a 20-foot fall hazard and were not trained in the recognition of fall hazards.

Employees were exposed to a 20-foot fall hazard.
