29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.         a) On or about may 10, 2021, employees were exposed to the hazard of falling from elevations of up to approximately 20 feet while working from an aerial lift without fall protection.   No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) On or about May 10, 2021, employees were exposed to the hazard of falling approximately 30 feet while working without fall protection from a pitched roof.  Group XL, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1443991, citation 1, item 1a, and was affirmed as a final order on January 14, 2020 with respect to a worksite located at 1421 Milwaukee Ave., Glenview, Illinois.   No abatement certification or documentation required for this violation.
