Charm Builders, Ltd. was previously cited for a violation of an equivalent standard (Cited as 1926.501(b)(10)], which was contained in OSHA inspection number 1426866, citation number 1, item number 1 and was affirmed as a final order on 12/12/2019, with respect to a workplace located at 1720 Jane St., Pittsburgh, PA.  Charm Builders, Ltd.  was previously cited for a repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1253476, citation number 2, item number 1 and was affirmed as a final order on 9/4/18, with respect to a workplace located at 47 2nd St NE, Massilion, OH.  This violation was predicated by a willful violation of this occupational safety and health standard, which was contained in OSHA inspection number 783421, citation number 1, item number 1 and was affirmed as a final order on 8/14/13, with respect to a workplace located at 205 W. 4th St, Port Clinton, OH.

(b) 1021 Mount De Chantel Rd, Wheeling, WV - On or about May 12, 2021, employees were accessing the roof using an aluminum extension ladder which did not extend at least 3 feet above the upper landing surface, exposing employees to fall of up to 11 feet.
