29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  WellNow Urgent Care center project, 502-514 2nd Street, Fulton, NY 13069:  On or about 5/13/2021, three employees were sheathing a low slope roof at sixteen feet above the lower slab, without fall protection of any kind.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)  WellNow Urgent Care center project, 502-514 2nd Street, Fulton, NY 13069:  On or about 5/13/2021, an employee was observed using a ten foot step ladder to egress the upper deck.  The worker climbed down between trusses, then lowered himself onto the upper level of the ten foot step ladder that was two feet below the bottom of the truss.
