29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to ensure that employees are not exposed to fall hazards.  This was most recently documented on May 4, 2021 at the jobsite located at 9646 Topher Boulevard in Lincoln NE. At least three employees working on the roof of a new construction home were not utilizing fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to ensure that employees are not exposed to fall hazards. This was most recently documented on May 4, 2021, at the job site located at 9646 Topher Blvd in Lincoln, NE. The employer failed to ensure that where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails were extend at least 3 feet (.9 m) above the upper landing surface being accessed.

This was most recently documented on May 4, 2021, at the job site located at 9646 Topher Blvd in Lincoln, NE. The area around the bottom of ladders was not kept clear; there were a lot of plywood in that area.
