29 CFR  1926.501(b)(1):Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system  (a)Bridge deck; On or about May 11, 2021 employees were working above 6 feet while removing wood from the outer edge of the bridge without a means of fall protection.

29 CFR  1926.502(b)(2):Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  (a) Bridge deck; On or about May 1, 2021 the employer did not ensure that the guard rail system constructed on the bridge deck was provided with a midrail, exposing employees to fall hazards.
