29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.  On the 4:12 pitch roof at the north end of condominium building 405, located at 405 S Villa San Marco Drive, Saint Augustine, FL: On or about 5/18/2021, the employer exposed employees to fall hazards ranging from 9-foot 7-inch to a lower roof and 19-foot 3-inch to the ground, in that fall protection was not utilized by employees while performing roofing work.  b.  On the 4:12 pitch roof at the north end of condominium building 405, located at 405 S Villa San Marco Drive, Saint Augustine, FL: On or about 5/18/2021, the employer exposed employees to a 9-foot 10-inch fall hazard to the ground, in that fall protection was not utilized by employees while performing roofing work.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the north end of condominium building 405, located at 405 S Villa San Marco Drive, Saint Augustine, FL: On or about 5/18/2021, the employer exposed employees to a 9-foot 10-inch fall hazard, in that a 12-foot portable aluminum extension ladder did not extend at least 3 feet above the landing surface while employees gained access to the lower roof.
