29 CFR  1926.451(b)(2):Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  On or about May 19, 2021, employees were exposed to a fall of approximately 14 feet while performing masonry activities at 34700 Madison Lane, Solon, Ohio 44139.  The employees were using a walkway to move from one scaffold to the other and the employer did not ensure that it was 18 inches wide.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about May 19, 2021, employees were exposed to a fall of approximately 14 feet while performing masonry activities at 34700 Madison Lane, Solon, Ohio 44139.  The employees were working from a supported scaffold and the employer did not ensure that they were protected from falling by an adequate guard rail system.
