29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (a) 4965 Carlisle Pike Mechanicsburg, PA  - Two  employees were utilizing a JLG 460-J aerial lift to access the exterior fa�ade of the building approximately 15 to 16 feet high and were not wearing fall harnesses, exposing the employees to a fall hazard, on or about May 19, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
