29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: Worksite located at 1317 Independence Ave., Kansas City, Missouri- Employees were in the process of removing the existing deck off the roof and had created holes in the roof that could cause the employees to fall from the second level rooftop to the first level.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Worksite located at 1317 Independence Ave., Kansas City, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the 2nd level to the roof level was not extended at least 3 feet above the upper landing surface.
